PHC is an acute, in-home rehabilitation healthcare provider, serving primarily the
Geriatric population. Patients typically have multiple co-morbidities and require
complex interventions in the home to maintain independence and quality of life.
Established linkages with Center for Medicare and Medicaid Services to enhance
Corporate response to Federal regulatory health care changes. Established preferred
Provider status with several major players to establish and maintain a sound referral
